Your player or coach MUST arrive 30 minutes BEFORE the end of the late add event to be processed at that event.

All DOCUMENTS WILL NEED TO BE PHYSICAL COPIES
NO ELECTRONIC VERSIONS WILL BE CONSIDERED OR ACCEPTED

REMINDER:  Players are only eligible after completing their 10 hours of conditioning practice AND 10 hours of additional practice (20 hours total).

Players Will Need:
- To be added to Affinity (with conforming photo) before arrival
- Conforming photo uploaded to Google Doc by NOON the day before late add event
- Birth Certificate
- Parent-Player Contract (front and back, signed by both)
- Medical History Form
- Current Physical that does not expire before Dec 15,2022 
- Full year report card for 2021-2022 school year
- BOTH Home and Away Jerseys (FB only)

Football Coaches Will Need:
- To be added to Affinity (with conforming photo) before arrival
- Conforming photo uploaded to Google Doc by NOON the day before late add event
- Certs and pictures uploaded to Affinity
- 2022 USA Football Cert
- 2020, 2021, or 2022 Mid-America Cert

Spirit Coaches Will Need:
- To be added to Affinity (with conforming photo) before arrival
- Conforming photo uploaded to Google Doc by NOON the day before late add event
- Certs and pictures uploaded to Affinity
- 2022 YCADA Cert or 2021 YCADA & 102 Course Cert
